The Connecticut Health Foundation’s (CHF) 10 year strategic plan included a goal to decrease the number of children and youth (ages 6-14) from entering the Juvenile Justice System and/or the most intensive level of mental health treatment. To accomplish this goal, CHF created a grant program to support and promote a comprehensive community-based system of early screening, assessment, identification, and brief intervention. Eleven Connecticut communities have been selected to receive an eighteen month grant to create a strategic plan for implementing a comprehensive community mental health program. EDC has been selected to provide training and technical assistance to the eleven grantees. We will provide a guideline for creating a strategic plan for systems change that is accompanied by trainings and site visits. The strategic planning for systems change is broken into components that will include: creating logic models; needs assessment; situation analysis; leadership; partnership; communications; cultural and linguistic competency; community outreach and engagement; and creating plans for sustainability and alternate means of revenue.
